Effective date: 06/17/2026

This Privacy Policy explains how Childhood Health Associates of Salem (“CHAOS,” “we,” “us,” or “our”) handles information collected through our website at https://www.childhoodhealth.com (the “Site”). Please read it together with the important notice below about protected health information.

This policy does not cover your medical information

This Privacy Policy applies only to our website. It does not govern the protected health information (PHI) we maintain as your child’s healthcare provider. The privacy of your child’s medical records is covered separately under our Notice of Privacy Practices and applicable law, including HIPAA.

Please do not send medical information, test results, or other sensitive health details through this website or by email. To communicate securely with your child’s provider, use our patient portal or call our office at 503-364-2181.

What we collect

We have built this Site to collect as little information as possible.

  • We do not use advertising trackers, marketing pixels, session-recording tools, or third-party website analytics. We do not sell or rent any information about visitors.
  • Server logs. Like virtually all websites, our web host automatically records standard technical information when you visit, such as your IP address, browser type, and the pages you view. This is used only for security and to keep the Site running properly, and it is retained for a limited period by our hosting provider.
  • Cookies. Our Site uses only the essential cookies needed for basic functionality. We do not use cookies to track you across other websites or for advertising.

Links to other services

Our Site links to outside services that are operated by other companies and governed by their own privacy policies, not this one:

  • Patient Portal (InteliChart) — used to request appointments, message your provider, view records, and pay bills. When you click through, you leave our Site and are subject to InteliChart’s privacy practices.
  • Social media — we maintain a Facebook page; activity there is governed by Facebook’s policies.

We encourage you to review the privacy policies of any third-party service before using it.

Children’s privacy

This Site is intended for parents, guardians, and caregivers — not for use by children. We do not knowingly collect personal information from children through the Site. If you believe a child has provided information through the Site, please contact us and we will delete it.

Your choices and rights

You may browse the Site without providing any personal information. Depending on where you live, you may have rights to access or request deletion of personal information we hold about you. Because this Site is informational and collects so little, in most cases we hold no personal information about visitors to access or delete.

Changes to this policy

We may update this Privacy Policy from time to time. When we do, we will revise the effective date above.

For the privacy of medical records and protected health information, please refer to our Notice of Privacy Practices.
